ATHLETIC CLEARANCE
High School Student Athletic Procedures (Student Forms)
The following requirements must be completed PRIOR to registration of all student-athletes:

EL2 Physical Form
(Page 4 of 4 must be completed which includes: signed, stamped, dated, and cleared without limitations by approved medical personnel. The supplement page may be required.) -
EL1 Electrocardiogram (ECG) Screening
(Effective July 1, 2026, all first-time high school participants in FHSAA athletics must have an Electrocardiogram (ECG) screening before participation. This applies to students with no cardiac symptoms. Students with cardiac symptoms should consult their healthcare provider. An ECG completed within two (2) years prior to July 1 of the participation year satisfies this requirement. If the ECG requires further evaluation, the student must be cleared by a licensed medical practitioner trained in the diagnosis, evaluation and management of ECGs before participating in FHSAA athletic competition, practice, tryouts, or workouts.) -
Four (4) required FHSAA Videos
(Concussion for Students , Sudden Cardiac Arrest, and Heat Illness Prevention, and Sportsmanship)
(print all three certificates in student-athlete’s name, dated after May 15, 2026) -
Enrollment & Residential History Form (print and complete)

Purchase Insurance (print insurance card)

Government Issued Photo Identification of parent/guardian who is signing the forms for the student-athlete - address must match Proof of Residence

Proof of Residence (example: utility bill dated within the last 30 days, mortgage, lease agreement, tax bill, etc.) in the parent/guardian's name

Birth Certificate of Student-Athlete
Once all of these documents are collected and ready for upload, please complete the online registration for your student-athlete at Athletic Clearance FHSAA.
